docs: warn about URL-controlled bucket selection in the loader

When the loader bucket config is empty, the first path segment of the
request selects the bucket. Combined with ALLOW_UNSAFE_URL this lets
anyone read from any bucket the loader credentials can access, so
document the risk and recommend scoped IAM policies.
