Plan 2.8: rewrite optimizer.md and update architecture.md cross-references

optimizer.md is replaced wholesale with a short, current-state-only spec:
provider-trusted cooldowns shared via the journal (no state store), opaque
scope, per-(model, operation) quality windows with no global verdict, the
journal as the only derived-state mechanism, sync(preset) seeding, demoted-
last selection, and log-line-only visibility (no alerts API, no status enum).

architecture.md is updated throughout to match Plan 2's actual current state:
three broker ports (registry/secrets/knowledge, not four — StateStoreProtocol
is standalone and unused by the broker), sync(preset) replacing SeedPolicy
and model CRUD, plain scope replacing user_id-everywhere multi-tenancy,
knowledge-store defaults and DB schema (fresh-create-or-fail-fast, no ALTER
migration; llmbroker_disabled table; no profile column), and the plain
help/extra key-help passthrough replacing the effort/value taxonomy.
