Merge dev into main (#62)

Promotes dev into main with the latest transport/reliability fixes. This
fixes reliable/ordered acceptance and false ACK behavior, best-effort
delivery for custom-named pinned CAN topics, OOM cleanup paths, reliable
response OOM handling, and subscription extent aggregation. It also
removes vestigial publisher auto-cleanup/topic-count code and adds
regression coverage for future teardown/reaping and pinned-topic
delivery.
