Fix CI: formatting, simd example gating, doxygen header, pre-commit config, blog-md

- clang-format + gersemi (CMake) formatting across the tree
- Guard examples/simd_example.cpp so it builds without std::simd (real code
  only at C++26 with <simd>, no-op otherwise); fixes the gcc-debug/gcc-release
  preset build failures
- Blank Doxyfile HTML_HEADER, which pointed at a missing docs/header.html
- Merge the duplicate top-level 'exclude' in .pre-commit-config.yaml (restores
  papers/wg21 exclusion; fixes check-yaml and checkmake)
- papers/blog: add blog-md GFM target (+ test phony) and generated .md
