chore(test): Fix incorrect coverage suppression in spec. (#470)

The pragma: nocover directives are artificially supressing cases where
testing is needed.

Some of these cases were fixed by unifying handling for missing and invalid
headers where the fallback behavior is the same.

23 of 23 new or added lines in 2 files covered. (100.0%)
