refactor: tighten VecProgress quorum coverage

# Summary

Expand VecProgress quorum tests while reducing production access to
helpers that are only needed by test code.

# Details

Add reference-model coverage for monotonic updates, randomized quorum
upgrades, joint and shrunk quorum transitions, learner and non-member
edge cases, exact stats behavior, and custom quorum-set semantics.

Use the fallible progress lookup when checking self progress before
mutation, so missing progress remains explicit and read-only checks do
not require mutable access.

Gate test-only VecProgress accessors behind `cfg(test)` and keep the
production surface focused on the methods used by runtime code.
