fix: include reports and warnings tables in sqlite-to-postgres converter (#414)

The converter hardcoded four tables and silently dropped reports and warnings,
so a migration to postgres lost the report history that drives report.autoban
and the warn auto-ban counters. Convert both tables with the same id-width
fixes as their native postgres schemas and handle the notification_sent
boolean in data rows like detected_spam.added.
