fix: make CENC fragment encryption CMAF-conformant

- cenc with an 8-byte input IV now produces 8-byte per-sample IVs
  (tenc.DefaultPerSampleIVSize=8, +1 per sample per CENC 9.2), the
  size CMAF 8.2.3.1 requires; 16-byte IVs keep the old behavior
- omit senc/saiz/saio when a fragment has no sample auxiliary
  information (cbcs constant-IV full-sample), per CMAF 8.2.2.1, and
  accept such fragments in DecryptFragment
- keep SencBox subsample entries aligned with the sample index so
  mixed fragments encode a zero subsample count instead of panicking;
  saiz sizes now match the senc layout, uniform sizes collapse into
  default_sample_info_size, and AddSampleInfo returns an error for
  sizes above 255 instead of panicking
- protect-range functions return an all-clear entry for degenerate
  samples so senc/saiz stay consistent within a fragment
