feat(bb_sub_mqtt): report ingress liveness into bb_transport_health (inferred, observe) (#686)

Mirror the egress sink's bb_transport_health wiring for the MQTT ingress
adapter: lazily register an INFERRED "mqtt_sub" slot on the first genuine
(non-self-filtered) inbound frame and mark_activity per frame, so every
bb_sub_mqtt consumer gets ingress liveness with zero app-side code.
