feat(server): improve history tombstones with metadata (#9632)

* Use minimal delete tombstones without Provenance per FHIR spec

Store minimal delete tombstones in _History with meta.deleted and other metadata.
Delete history entries return HTTP 410 as required
by the FHIR specification.

* Move restore test helpers into repo.test.ts

These helpers were only used by tests and did not wire up any runtime
restore operation, so colocate them with the test that exercises them.

Add parseHistoryContent helper and use it in readHistory, readVersion,
and search result hydration to avoid double JSON.parse calls.

* Simplify parseHistoryContent and remove unused isDeleteTombstone

Address review feedback: parseHistoryContent now returns Resource
directly (no null, no one-off ParsedHistoryContent type), callers
branch on parsed.meta?.deleted, and the now-unused isDeleteTombstone
helper and its test are removed.
