feat(server): improve history tombstones with metadata (#9632)

* Use minimal delete tombstones without Provenance per FHIR spec

Store minimal delete tombstones in _History with meta.deleted and other metadata.
Delete history entries return HTTP 410 as required
by the FHIR specification.

restore operation, so colocate them with the test that exercises them.

* Simplify parseHistoryContent and remove unused isDeleteTombstone

Address review feedback: parseHistoryContent now returns Resource
directly (no null, no one-off ParsedHistoryContent type), callers
branch on parsed.meta?.deleted, and the now-unused isDeleteTombstone
helper and its test are removed.
