release: django-logic 0.4.0

Cut the first PyPI release since 0.1.6. Fold the post-0.4.0 Unreleased
work (issues #98, #100) into the [0.4.0] changelog section, dated
2026-07-02, and document the #103 domain-outcome test helpers
(assert_journey / assert_changed / expect_raises / assert_transition_owner
and the failure-hook assertions) in README + docs/TESTING_GUIDE.md.

- pyproject version stays 0.4.0
- .gitignore ignores .pypirc so PyPI credentials never get committed
