fix(clickhouse): parse timestamptz default offset (#865)

Postgres renders timestamptz defaults with a UTC offset such as
'2026-01-01 12:30:00+00'. ClickHouse toDateTime64 string parsing
rejects the offset, so ADD COLUMN failed during initial copy.

Render these defaults with parseDateTime64BestEffort, which accepts
the offset and normalizes to UTC. Add a regression test for the
offset form.
