Add ascat.cf_conversions; rework ragged_array CF representations

Extract the CF discrete-sampling-geometry conversions into a standalone,
class-free module and harden the ragged_array wrapper classes, with CF-
correct multidimensional representations and full test coverage. Breaking:
the "orthomulti" API is replaced (no deprecation aliases).

New module (ascat/cf_conversions.py):
- Dataset-level conversion functions (point/indexed/contiguous/incomplete/
  orthogonal), callable directly on an xarray.Dataset with explicit dim/var
  names, with no dependency on the wrapper classes. cf_array.py re-exports the
  point/ragged functions for its accessor classes.

Shared primitives:
- Move fill_value, vrange and pad_to_2d into ascat.utils and import them
  everywhere (previously duplicated across ragged_array.py and
  ragged_array_ts.py); drop the duplicate dtype_to_nan in xarray_io.py in
  favour of the canonical one in ascat.utils.
