feat: handle server_tool_use / server tool_result / fallback blocks

Modern Claude transcripts carry server-side tool_use (e.g. webReader /
web search), their inline tool_result, and fallback (redacted) blocks in
assistant content. claugrep warned 'skipping unrecognized assistant
content block type' on every one, flooding stderr on /.

Now: server_tool_use surfaces as a ToolUse (searchable via -t tool-use,
name = server tool), the server tool_result surfaces as ToolResult, and
fallback is skipped silently. Any genuinely-unknown block type now warns
at most once per type per process instead of per record. Added a parser
unit test covering all three.
