feat(llm, anthropic): Add `claude-sonnet-5` model support (#820)

Users can now select Anthropic's `claude-sonnet-5` model. `jp init` also
defaults newly initialized Anthropic workspaces to `claude-sonnet-5`.

The new model supports a 1M token context window, 128k max output
tokens, adaptive thinking, interleaved thinking, and context editing.

While here, `claude-sonnet-4-6`'s max output tokens is corrected from
64,000 to 128,000 to match the provider's published limits.
