Remove cursor, add fish tests, CLI --format flag, README, go.mod bump

- Remove SplitForCompletionAt (cursor param was ignored); merge into
  single SplitForCompletion(s, format) function
- Add format_fish_test.go with 11 tests covering \'/\ in single quotes,
  keyword operators (and/or/not), open quote, pipe, semicolon, completion
- Add --format flag to CLI (bash/zsh/fish/elvish/nushell/powershell/
  xonsh/tcsh/oil/cmd) and --completion-context flag
- Add carapace replace directive to go.work and cmd/go.mod so CLI builds
  against local carapace with Span.Start fix
- Rewrite README.md with v2 API docs, format table, CLI usage, token model
- Bump go.mod from go 1.15 to go 1.24.0
- Update architecture.md and plan.md to remove SplitForCompletionAt
