testing: switch Vitest config to threads for most packages (#9667)

* Switch Vitest pool to threads for most packages

Vitest defaults to forks for stability, but threads reduces worker
startup overhead and improves test run times across the monorepo.
Packages with serial execution or native-module concerns (agent, hl7,
server) are left unchanged.
