docs: changelog and migration notes for the v2 API cleanup sweep

Document the §1 polish sweep in CHANGELOG [Unreleased] and MIGRATION.md:
integrity option aliases, package-prefixed error sentinels, driver pointer
receivers, per-instance dummy lock registry, KeyType.String wire form,
exported algorithm-name constants, unexported RSAPSS/ModRevisionEmpty,
predicate string normalization, etcd NewWithLocker, tcs DoerWatcher→Client,
unified locker name validation, and namer.Results value receivers.
