Simplify banner/divider comments in tests and public headers

Mechanical comment-only sweep across the curve/quadrature test files and
the public API headers: drop section-banner lines (// --- X ---),
// ===== Name ===== divider rails, and review-process residue
(B-new-N / BAR-X labelled comments that reference the critique/spec
process rather than the current code).

Phase 1 of the codebase-wide 'explanation belongs in docs, not raw code'
effort. No code is changed; only comment and blank lines are removed.
dal-cpp/dal/auto/, dal-excel/auto/, dal-cpp/externals/, file headers,
and Machinist markup are untouched.
