address audit follow-ups (maintenance, refresh, expiry)

- maintenance: run afterWrite's inline assisting task in a finally, so a
  drain that throws (broken key hashCode) can't drop the write's policy
  update (weightedSize drift / entry invisible to eviction). The trigger
  is a user Map-contract violation, so this is robustness.
- refresh: pin that a same-instance refresh clears its token and a later
  read refreshes again — refutes a suspected token leak (false positive).
- expiry: document read-path expiry extension as best-effort — a just-
  expired entry can briefly resurrect, inherent to lock-free read-
  extension over lazy expiration (accepted, won't-fix).
