test: add comprehensive unit tests for middleware and error handling

- Add consumed-capacity-middleware.test.ts with extensive unit tests covering:
  * Basic functionality of middleware chain
  * Edge cases: undefined values, missing keys, array responses
  * Error handling: propagating errors from next handler and callback
  * Middleware chain integrity and response preservation
- Expand DynamoDbRepository.unit.test.ts with additional tests:
  * Empty response handling for both batchGetItems and batchWriteItems
  * Multiple retry scenarios for UnprocessedItems
  * Error propagation from DynamoDB operations
  * Edge cases like missing Responses table keys

These tests improve code confidence and catch potential issues early.
