Fail fast on v0.4 profile configuration conflicts (#128)

* Fail fast on v0.4 profile configuration conflicts

Reject published v0.4 directional-PCA profiles when their requested fanout conflicts with configured materializability constraints, while preserving emergent child-count underfill and legacy published-profile behavior.

Update the streaming-indexer published-profile construction path, spec validation coverage, and the requirements/design/validation spec package to reflect the scoped fail-fast contract.
