fix(mock): don't reuse ID 123 for two Patients (#9611)

* fix(mock): assign unique ids to seeded Practitioner resources

Mock lookup tables key rows by resourceId only, so Patient/123 and
Practitioner/123 collided and overwrote HumanName index rows during seeding.
Move Dr. Alice Smith to Practitioner/124 and export well-known mock id constants.
