docs(swagger): document integrator and api-key endpoints and scopes

Make the swagger for the integrator and chain-abstraction endpoints correct and
comprehensive, verified against the handlers:

- BearerAuth scheme now states it accepts a JWT or a scoped "vsk_" API key.
- POST /organizations: document the provisionAccount and integrator opt-ins and
  the free-integrator-plan 404 case.
- POST/PUT /process: document the additive electionParams; fix the update body
  type to UpdateProcessRequest; add the 403 (draft plan limit) and 409 (already
  published) responses.
- publish / vote / status / managed-org / integrator-info: note the required API
  key scope (voting:write, managed:write, managed:read, quota:read) and clarify
  quota/limits semantics.
- API keys: list the valid scopes and secret-once behavior on create; add the
  missing 400 on revoke.

Regenerated docs/swagger.yaml.
