docs(queue janitor): clarify stale scan is limit-bounded, not scan-bounded

Reword the cleanup comment and list_stale_processing_ids/3 docs to make clear
that @stale_processing_limit bounds the stale rows acted on per pass, not the
rows ETS scans: the table is not indexed by claimed_at, so a select may walk
further to find matching stale rows.
