Add recursive planning progress telemetry (#119)

* Add recursive planning progress telemetry

Improve streaming-indexer observability for long-running recursive and divisive planning by extending structured status updates with current-unit descriptors, recursive counters, and live hierarchy heartbeats. Update the streaming-indexer specs and add validation coverage for slow recursive progress emission and published-profile directional PCA telemetry.

* Harden hierarchy heartbeat ordering

Prevent stale hierarchy heartbeats from being emitted after a unit advances or completes by tagging snapshots with a generation token, and bump lexongraph-streaming-indexer to 0.2.0 to reflect the public status-struct expansion.
