fix(server): render CMS-1500 Box 24E diagnosis pointers (#9595)

The CMS-1500 PDF exporter hard-coded Box 24E (diagnosis pointer) to an
empty string, so every generated claim form omitted the per-line link
between service lines and the Box 21 diagnoses, even when the Claim's
item.diagnosisSequence was populated.

Render the pointers from item.diagnosisSequence as reference letters
(1 -> A, 2 -> B, ...), entered with no separators per the NUCC CMS-1500
instructions (e.g. "AB"). Extract a small pure formatDiagnosisPointers()
helper and unit-test it alongside the other formatters.
