fix: handle nil mailing_list_consent as no-op

Previously nil (falsy) mapped to unsubscribe, same as false. Use
explicit case/when so nil consent does nothing — only true subscribes,
only false unsubscribes. DB default is false so this only matters for
rows created before the default was added or programmatic creation
without the attribute.
