fix: Form designer back button should return to previous user-facing page, not hard-coded /forms (#692)

# fix: Form designer back button should return to previous user-facing
page, not hard-coded /forms

## Description
- After analysis, fix was to return to the Forms details page to avoid
passing query params or using other insecure approaches like session
store.
- Added improvements to the forms page to show the folder of the form
- Fixed issue with the folder's breadcrumbs being rendered on other
pages like submission list and details
