fix: correct IOUT_OC bit (b4→b7) and clear burst tracking on hold-clear (#520)

- tps546_decode_fault_bits: check STATUS_IOUT bit 7 (IOUT_OC_FAULT) not
  bit 4 (IOUT_UC); fixes false-negative during the .80/650 board OC incident
  where STATUS_IOUT=0xA0 (bits 7+5 set, bit 4 clear) was not decoded as OC
- fix macro comment: STATUS_IOUT bit 4 → bit 7 (IOUT_OC_FAULT)
- bb_vcore_wd_clear_hold: also reset burst_count/burst_window_start_ms so
  the first recoverable collapse after a hold-clear returns RECOVER instead
  of BACKOFF (stale burst_count at/over BURST_MAX silently blocked recovery)
- tests: update OC fixture from st_iout=0x10 to 0x80; add incident vector
  0xA0 test; add UC-does-not-trigger-OC regression guard; add
  clear_hold_resets_burst_tracking test
