refactor: single descriptor table for publisher-health gauges (B1-326) (#499)

- introduce pub_gauge_desc_t table (k_pub_gauges_base + k_pub_gauges_buf)
  with suffix, json_key, and is_bool per gauge
- add pub_gauge_snapshot() to collect all live values once per handler call
- drive all three emit sites (prom, json values, schema/json) from the table
- adding a gauge is now a one-row edit in the descriptor; no other changes
- emitted prometheus/json output is byte-identical; all 2412 host tests pass
