refactor(color): finalize threshold API and add CLI support

- color-first arg order (ColorAbove(color, threshold)) to match other *Color options
- thresholds take precedence over gradient; both bounds exclusive (>/<)
- add -ca / -cb CLI flags
- README example + screenshot, trimmed docs, test updates
