fix(premium): finalize proxy enrollment in-join with offline uuid mode

canBypassWithPremium now mirrors the v4 path for v3/proxy: it peeks the
queued perform.login request and finalizes the enrollment in-join, using
pendingPremiumCache.removePending as the atomic one-shot gate it already
shares with ProxyLoginRequestValidator. Whichever path wins the gate
finalizes; the loser stays silent, so there is no double finalization or
spurious "premium verification failed" message.
