Provider: Streamline scheduling encounter creation (#9373)

* Provider: Add HealthcareService scheduling configuration page

First step: control over one extension in the HealthcareService
resource. This will be used to streamline the Encounter creation step
in following commits.

Implementation Notes
- Added a new "Scheduling" tab to the ResourcePage for the
  `HealthcareService` resource type when the `scheduling` feature is
  enabled in the current project.
- Added a `useResource` wrapper for use within the provider UI that
  handles some state tracking and error display.

The first implementation was popping the extension out and re-inserting
at the tail of the extensions list, which made the history view more
difficult to read.

Let's adjust this to do an in-place replacement.

* Tighten "appointments" types

When calling `$book`, it can only return a single appointment. Let's
make our types reflect this.

* Push encounter creation code down into BookAppointmentForm

The `FindPane` component is getting pretty complex. Moving this new code
down into the Booking component instead of having it in a callback
should help keep things a little more local and make testing easier.
