fix: Resolve clippy warnings and classify Transfer-Encoding prefix as Ambiguous

Replace deprecated compare_and_swap with compare_exchange, add
rustfmt::skip for generated tables, and fix all clippy lints for
compatibility with current stable Rust.

Use extern "C-unwind" for public FFI functions to allow panic
unwinding across the ABI boundary, fixing should_panic test behavior.

Fix invalid enum discriminant handling by using integer comparison
instead of pattern matching when reading external header tier values.

Add classification logic to detect requests with ASCII characters
following 'Transfer-Encoding' header name. These are now classified as
Ambiguous to prevent HTTP desync attacks using obfuscated TE headers.

Replace deprecated grcov workflow with cargo-llvm-cov.

62 of 62 new or added lines in 3 files covered. (100.0%)
