test(rpc_dart): verify per-stream controllers drain; expose count in health

Adds a leak regression test for the per-stream routing in RpcChannelTransport:
asserts the controller map drains to zero after many unary calls (client and
server), on subscriber cancel, on end-of-stream, and on transport close, and
that getMessagesForStream is single-subscription. Exposes the live controller
count via health().details['streamControllers'] for production leak
monitoring.
