fix(observations): address PR #1680 review feedback

- Add dedicated migration file for substance column and QK consolidation
- Revert 9_04 and 9_05 SQL to match develop (no inline DDL/DML changes)
- Guard against null quantity kind lookup in update controller
- Use nullish coalescing (??) consistently for substance field
- Add explicit substance: null to fixture #4 for clarity
- Add nullable: true to Swagger SensorConfigurationCreate.substance
- Fix FK violation: make constraint DEFERRABLE before ID reassignment
- Fix stale denormalized quantity_kind_code in t_time_series
