fix(server): configurable lobby body-parser limit and documentation improvements (#29)

* docs: document breaking change in 0.50.4 (koa-socket-2 removal)

- Add agent-facing doc with architecture analysis
- Add user-facing CHANGELOG entry with migration guide
- Include example code for origins callback signature

* fix(server): make lobby API body-parser limit configurable, default to 5mb

koa-body defaults jsonLimit to 1mb, causing 413 errors for large setupData.
Create a single reusable body parser inside configureRouter with both
jsonLimit and formLimit driven by a new apiBodyLimit option (default '5mb').
Apply it consistently across all POST routes.
