fix: force synchronous sub-agents in child claude (#16)

Claude Code 2.1.x backgrounds Agent/Task sub-agents by default. Under fya's single ephemeral PTY turn the main turn ends while the agents run detached, and fya tears down the PTY before they finish, so orchestrators relying on parallel sub-agents (e.g. ralphex's review phase) get empty results.

Default CLAUDE_CODE_DISABLE_BACKGROUND_TASKS=1 on the child unless the caller set it, forcing foreground/synchronous sub-agents that complete within the turn - the headless `claude -p` behavior fya stands in for.
