feat: check_on_apply heap-fallback directive for boot-mode OTA (#456)

- add BB_UPDATE_OUTCOME_CHECK_ON_APPLY enum value to bb_update_check_outcome_t
- add bb_update_check_mark_check_on_apply() to mark status as deferred-to-apply
- add "check_on_apply" case to outcome_str() in bb_update_check_common.c
- add CONFIG_BB_OTA_CHECK_ON_APPLY_FALLBACK Kconfig (depends on BB_OTA_BOOT_STATUS_HTTP, default n)
- update ota_boot_check_handler heap-guard branch: when fallback enabled, return 200 {"status":"check_on_apply"} instead of 503 {"error":"insufficient_heap"}
- extend s_status_responses outcome enum to include "check_on_apply"
- add 200 entry to s_check_responses documenting the directive
- add host tests: test_emit_status_check_on_apply, test_bb_update_check_mark_check_on_apply_sets_status, test_bb_update_check_mark_check_on_apply_before_init_returns_invalid_state
- document check_on_apply directive in CLAUDE.md OTA strategy section
