test(cache): cover blocking()/terminated(); simplify getDb return

Address review:
- Add specs that fire versionchange/close on the live connection and
  assert blocking() closes + reopens and terminated() reopens. Both fail
  if the handler is removed.
- Drop the redundant `|| null` in getDb (the catch already yields db-or-null).
