fix: address review comments - doc clarity and unmount assertion

- Update ensureMountPoint doc comment to accurately describe all return
  value combinations: (true, err) when mount is present but unhealthy
- Add Unmount override to fakeMounter with unmountCount tracking
- Assert in unit test that Unmount is NOT called when shouldUnmount=false
