refactor: replace delete-host list modal with InputField in footer row

Replace the full-screen tview.List modal for 'd' (delete host) with an
InputField in the footer row, matching the pattern used by 'a' (add host)
and '/' (filter). The user reads the host name from the visible Ping Monitor
table and types it directly into the input, keeping the existing view
unchanged while deleting. This removes rootPages and deleteList entirely,
simplifying the UI layer.
