feat: add herdr launcher backend and fix untracked --include/--exclude (#228)

adds a herdr terminal-multiplexer backend to all three launchers (before kitty) so /revdiff and plan review work natively inside Herdr, and fixes untracked files bypassing --include/--exclude via diff.FilterPaths + filterUntracked. Includes launcher test coverage (fake herdr fixture, HERDR_* env isolation), the jq-then-grep id-parse fallback, and docs.
