  fix: repair derived concepts and broken shipped configs

  Derived concepts could never run end-to-end:
  - dependency resolution iterated the characters of the table concept
    string and ignored join concepts, crashing the topological sort
  - input paths were built by splitting the identifier on dots, which
    never matched where simple concepts write their output; resolve via
    the registry instead
  - the subject_id/time event defaults were bare column names (invalid
    expressions) and missing extension columns crashed; defaults now
    mean pass-through
