Add release workflow that stages an npm publish on GitHub release (#931)

When a release is published, CI stages the package version with the
new npm stage publish command (with provenance) rather than publishing
directly; a maintainer then approves it with npm stage approve, which
is where the 2FA proof-of-presence happens. Requires an NPM_TOKEN
repository secret.
