* Add agentic engineering guide and companion blog post

New tutorial "Building on Medplum with AI Coding Assistants" covering
how to give AI tools access to Medplum, plan/prompt against it as the
source of truth, manage context decay, and verify output. Companion
blog post links to the guide.

Adds section 5 "Encode Your Conventions in a Rule File" with an example
AGENTS.md-style rule, a 5th recommendation, and cross-references from
the prompting, conversation, and verification sections.

* Add codes/terminology common-mistake to rule file

Note that agents hallucinate SNOMED/LOINC/ICD-10 codes; instruct using a
placeholder and flagging for human verification rather than inventing codes
or fetching them from the internet.
