app: serve sitemap dynamically so it is always current

Replace the boot-time file generation (which went stale between deploys and
differed per replica) with GET /sitemap.xml rendered from the DB per request:
always fresh, consistent across replicas, same URL. Removed the on-boot
sitemap:refresh and the committed static sitemap/config. The whenever and
sitemap_generator gems are now unused and can be dropped at the next bundle.
