release: 2.5.4 -- parameter-scoped tool grants + CI pinning + codebase_map/scip standalone fix

PATCH release per the versioning policy (additive opt-in feature +
fixes). Rolls up everything accumulated on main since 2.5.3:

- feat: parameter-scoped tool grants in allowed_tools (Tool(pattern)
  enforced natively on Claude + post-hoc verification, E073,
  on_grant_violation, tool_grant_violation event, has_scoped_tools)
- fix: context_mode codebase_map/scip work without context_from
- ci: tooling pinned via .github/requirements-ci.txt + Dependabot
- test: free Ollama real-engine coverage for the former "partial"
  areas; coverage restored to ~100%
- docs: ROADMAP reference hygiene (scrubbed non-citable source,
  fixed ContextBench link, stable arXiv URLs)

Version bumped in pyproject.toml, __init__.py, README, CLAUDE.md,
CODEX.md, docs/VERSIONING.md; CHANGELOG Unreleased promoted to 2.5.4.
