Align with rfc8216bis-22 and enforce EXT-X-TARGETDURATION minimum of 1

rfc8216bis-20 added the requirement that the EXT-X-TARGETDURATION value
MUST be at least 1. Floor the calculated value (calcNewTargetDuration) and
SetTargetDuration at 1 so a sub-second segment or an explicit 0 no longer
emits an invalid #EXT-X-TARGETDURATION:0.

Reviewed drafts 19-22: no new/removed tags or attributes and the version
compatibility rules are unchanged, so no other code updates are needed.
Update README to state alignment with rfc8216bis-22.
